PRESEASON / Before Practice:

A. Attend REGISTRATION (Apr. 27 or 30) with the following forms completed

1. (Cheerleaders only) Submit Measurements at REGISTRATION or by: _ May 8th of current year

B. Cheerleader Uniform Fitting (See Measurement Worksheet for details)

I. Submit Measurements at REG ISTRATlON or by: May 8th of current year

2. Attend Fitting Appointment; Approximate Date 3rd week of May TBD DATE

C. REGISTRATION CLOSES (for Cheerleading due to uniform orders and tailoring) _ May 31st of current year

D. A Sports Physical must be completed AFTER April 15th of this current year.

I. Other than your family doctor and insurance, one opportunity for a sport physical is at the annual

Frankenmuth High School Athletic Sport Physical [Fundraiser / "Practitioners-on-the-Run"]

Approximate Date: mid-July; July 19

2. Submit the Sport Physical document at the Parent Meeting, Uniform Handout or sooner.

3. Participation in practice: ONLY when the Sport Physical document is on file with Muth Youth.

E. Parent Meeting: (Most of the following TBD DATES will be defined at this meeting.)

I. Attend to receive important info about upcoming season July 20

F. Equipment / Uniform Handout

I. FOOTBALL Approximate Date: last wk. of July;

2. CHEERLEADING Approx. Date: 1st day of Practice

3. Equipment / Uniform WILL 0 LY be issued with 100% completion of forms

as listed in items IA & IB described above.
