Please bring your completed forms:

1. Player Conduct 2.Parent Conduct 3. Emergency Medical 4. Parent Waiver

5. (Copy of Birth Certificate if not submitted with registration)

6. Sports Physical Card

NOTE to parent / guardian:

Your child will NOT be allowed to practice until all six (6) forms are completed and submitted.

$20, proceeds are for the High School Athletic Program

In case you missed it, Muth Youth Registration was held on April 14 and 17th of 2010. Registration is closed.

* Coaches *

Each coach (and board member) has to submit their annual "Background Check Form". This is available from the following link.

(...for what it's worth, this information is handled / processed the same as what a police officer does during a "routine" traffic stop. Muth Youth does not process it - our Chief of Police provides this service)
